What the URL of the rest call look like? Is it to a file in a folder other than the web root? (such as proxy)
If so, you’ll need to set up your COLDBOX_APP_MAPPING in the application.cfc.

The REST file is inside the webroot, the same root where coldbox fw is.
Sample REST url: http://localhost/api/v1/index.cfm/address/validation

Folder structure:
webroot

__api

__v1/handlers/rest/Address.cfc

__ coldbox
