Did you make it through the ContentBox installer, or is this what you get as soon as you hit it?
It seems like a mapping issue at first blush. Can you tell us about your directory structure? Is ContentBox in the web root?

Hi Brad,
I made it through the installer and db verification. Once done it throws this error when I try to hit the index. My install is in a folder called contentbox in my web root. I’m using a MySQL db on OSX, Adobe CF 9.0.1

Thats the problem. Rename your enclosing folder to something else. Acf has a stupid bug with mappings that are named the same as the enclosing directory.
